COMMUNITY RELATIONS REPRESENTATIVE

POSITION DESCRIPTION
A hospice Community Relations Representative is a relationship manager, employed by Hospice. This professional networks with potential referral organizations throughout the community (e.g. hospitals, physicians, nursing homes) educating them on the benefits of hospice care in an effort to increase business.

ROLE EXPECTATIONS
The hospice Community Relations Representative actively reaches out to individuals and organizations within a region or territory, including churches, medical centers, nursing homes, community centers, industry associations and private physicians. The goal is to develop a network of potential business referral sources.

Clinical Competency
Builds and maintains relationships with clients through presentations and marketing. • Obtains the order to assess/admit to hospice and to request additional information as relevant to determine hospice appropriateness. • Is knowledgeable of conditions of participation for hospice diagnosis. • Follows Medicare guidelines and all applicable laws in regard to expenditures (e.g. physician lunches, gifts, etc.). • Meets with patients and families when requested to explain the Medicare hospice benefit. • Works in conjunction with the clinical team to support patient care delivery. • Supports an encouraging and healthy work environment. • Documents and submits expense reports on the date requested each month. • Maintains the integrity of company property (e.g. phone, computers, cars, etc.). • Demonstrates effective sales, communication and presentation skills.

Education

College degree preferred • Medical services/supplies sales experience preferred

RN CASE MANAGER

POSITION DESCRIPTION
A Registered Professional Nurse who, operating within the framework of the mission and philosophy of Hospice, delivers nursing care to patients. She/he utilizes the nursing process in a holistic approach to identify complex patients’ needs and to develop and implement creative strategies for meeting these needs. Demonstrates initiative in applying sound principles of management in planning, organizing, and delivering care. Provides care directly, supervises others, and serves as the manager of the interdisciplinary team. Collaborates and communicates with all members of the interdisciplinary team. Participates actively in meeting the goals and
objectives of the hospice organization. Performs and documents care adhering to the professional, hospice, regulatory body, and nursing standards. Demonstrates an ongoing commitment to her/his own professional growth.

ROLE EXPECTATIONS

Clinical Competency
Verbalizes knowledge of hospice and palliative care Nursing. Differentiates Hospice from acute care nursing as noted by verbal report during IDT meeting, and implementation of interventions supporting the hospice plan of
care.

Utilizes the Nursing Process in a holistic approach to identify complex needs and to develop and implement creative strategies for meeting those needs. Interventions and measurable outcomes are based on a thorough assessment of the patient, caregiver, and family.

Adjusts plan of care in response to the changes presented by patient, caregiver, family, or interdisciplinary team in a timely manner. Reevaluates the response to those changes by examining symptom improvement and patient, caregiver, or family verbal and nonverbal responses.

Recognizes, reports, and documents signs and symptoms of decline.

Documents data relative to complex or sensitive psychosocial, medical-legal, physiological states.

Develops, implements, and evaluates teaching plan. Incorporates teaching plan into daily care and documents.

Performs care adhering to the professional standards of practice and hospice standards of care.

Provides care mindful of the patient’s rights to confidentiality. Collaborates and communicates with the Case Manager. Participates actively in meeting the goals and objectives of the hospice organization. Performs and documents care adhering to the hospice, regulatory body, and nursing standards. Demonstrates an ongoing commitment to her/his own professional growth.

ROLE EXPECTATIONS

Clinical Competency

Delivers care to assigned patients following the direction and supervision of the Case Manager.

Observes, reports, and records information about the patient’s condition or change in condition accurately and timely to include but not limited to obtaining vital signs, observing the condition of the skin during bathing, and observing the pattern of elimination.

Maintains a clean, safe and healthy environment by providing light housekeeping duties for the patient and caregiver.

Acts as a patient advocate by observing the patient care setting, and caregivers’ interactions with the patient, noting safety and support of
the patient’s wellbeing. Communicates observations to the Case Manager.

Collects and/or leaves medical supplies for patient care as directed by the Case Manager.

Completes tasks listed on the Hospice Aide Plan of Care.

Documents the activities and observations of each visit and places documentation in the patient’s chart within the required timeframes.

Attends all required meetings to include but not limited to the weekly start up meeting and the bimonthly interdisciplinary team meeting.

Meets with assigned patients’ Case Managers at least weekly to discuss the care needs of the patient.

Education
A graduate of an accredited high school or equivalent.

SOCIAL WORKER

POSITION DESCRIPTION
Social Worker shall help patients and their families to enhance or restore their capacity for optimal psychological, emotional, spiritual, social and physical health. They identify coping skills and strengths utilized by the patient and family. Social Workers function as an integral part of the interdisciplinary team by addressing the psychosocial stressors and collaborating with the team to enhance the quality of end of life care for the patient and their family.

ROLE EXPECTATIONS

Clinical Competency
• Complete an initial psychosocial evaluation within the time limits set by the Conditions of Participation and/or state regulations.
• Demonstrate a working knowledge of the Conditions of Participation and state regulations governing hospice care.
• As allowed by patient and family, assist with advanced care planning to ensure that all treatments meet the wishes of the patient receiving care. Advanced care planning can include advanced directives, DNR’s, DPOA’s, financial planning and funeral planning.
• Assist the patient and family to understand, accept, and follow medical recommendations and provide services planned to assist the patient and family in achieving the optimum social and health adjustment within their capacity.
• Utilize available resources to assist the patient and family to live better within the limitations of the illness.
• Records and reports observed social and emotional changes within a timely manner.
• Demonstrates the ability to prioritize and balance daily schedules and completes duties in appropriate time frame.
• Complete tasks independently.
• Educate, mediate and problems solve with patient and families regarding ethical dilemmas that may arise.
